距骨天蓋の離断性骨軟骨炎

離断性骨軟骨炎はあらゆる関節で生じる可能性があり、距骨天蓋の場合は全体の4%を占める。1距骨天蓋における離断性骨軟骨炎では、しばしば足関節の痛みや障害の原因となる。2,3関節軟骨に生じる問題は、傷害に起因することが多いが、虚血性壊死や内分泌系疾患、遺伝的問題が原因である可能性もある。2,4-9しかし距骨天蓋外側に離断性骨軟骨炎がある場合、その原因は傷害であることが多い。10-13一方内側の場合、さまざまな要因が考えられる。11-13天蓋外側の傷害は、足関節の内反または背屈により、距骨天蓋の前外側が腓骨遠位部と衝突することで生じると考えられている。14
原因
離断性骨軟骨炎は足関節の捻挫に伴って発症することが多い。傷害に伴い距骨天蓋への血液供給が阻害され、組織の変性が進行する。そのため足関節捻挫の後、症状の改善が遅延している場合、距骨の離断性骨軟骨炎を疑う必要がある。
捻挫に伴い、距骨の関節面と脛骨、腓骨の関節面(天蓋)が擦れあうことで、これらの関節面に損傷が起こる。多くの場合、距骨関節面(骨軟骨)が損傷する。損傷部位は大きく分けて外側と内側に分けることができる。外側の場合、距骨天蓋の前側(つまり前外側)を損傷する傾向がある。また内側の場合、距骨天蓋の後ろ側(つまり後外側)を損傷する傾向がある。損傷部位の形状もそれぞれ異なる。距骨関節面の後内側は、足関節の内反に背屈が加わったとき、また前外側は内反に底屈が加わったときに発生する。14距骨関節面の内側に損傷がある場合、血管性または遺伝性の要因も考慮されるべきであるが、外側の場合は例外なく傷害性であると考えられている。10,15
症状
一般的に距骨天蓋の離断性骨軟骨炎は、患者の自覚症状が明瞭でないことが多いため、診断が難しいと考えられているが、症状は足関節の捻挫とほぼ同じである。急性期の症状の特徴は以下の通りである。10
- 関節の腫脹
- 内出血
- 自動的・他動的可動域の制限
関節可動域の制限は、骨軟骨断片が完全剥離している場合に生じる。これは骨軟骨断片が距腿関節の間で挟まることが原因である。また運動により、クレピタスが伴うこともある。
慢性期の症状の特徴には、深部痛や一過性の腫脹(休息により改善)があるが、自覚症状がまったく現れない場合もある。16深部痛は特に荷重位において自覚されることが多い。

検査法
触診検査では腫脹の有無を確認する。足関節の可動域検査では、関節可動域制限や運動に伴う痛みなどに注意して検査を行う。可動域制限は背屈方向に現れることが多い。また関節の不安定性検査も重要である。
距骨の離断性骨軟骨炎は、レントゲン検査において発見が難しいため診断が遅延する傾向がある。17-20そのため正しい診断がされるまでに4 ヶ月から2年を要することがある。17,21
治療法
治療法は症状の程度に応じて決まる。病変が軽度であれば、保存的療法によって改善が十分に見込めるが、軟骨下骨が露出している場合や骨折している場合などは、外科的手法に頼らざるを得ないこともある。一般的には、ステージⅠとステージⅡの場合、保存療法によって治療を行うことが推奨される。傷害の時点から6週間程度を目途に固定を行うと同時に、患者の症状改善具合を観ながら適度(10kgから20kg程度)の荷重をかける。治療を開始してから6週間後までには、X線やCTによる映像検査を行い、距骨天蓋の状態を確認する。患部の治癒が確認されたら、リハビリテーションを開始し、可動域の改善と筋力の増強、歩行運動の改善を行い、22症状改善の程度を勘案しながら運動を再開する。
治癒の経過はレントゲン検査によって評価することができる。18,19,23しかし距骨関節面の損傷の程度を評価するためには、MRIがより優れていると思われる。18,19,23
